Visiting OT Park & Covid-19
What is the vaccine requirement?
All visitors ages 12 and over must present proof of full vaccination or a negative COVID-19 test taken within 72 hours of your visit in order to enter OT Park facility.
Visitors ages 11 and under do not need to show proof of vaccine or a negative COVID-19 for entry.
You are considered fully vaccinated if it's been at least 14 days since your final vaccine dose. You will need to present your original vaccination card, a photo or copy of the card, or the California Digital COVID-19 Vaccine Record.
We accept any vaccine approved by the World Health Organization. Sputnik and CanSino vaccines are not accepted at the OT Park.
Visitors ages 18 and over will also be asked to present a photo ID.
If you’re not fully vaccinated, bring a negative COVID-19 test result (rapid antigen test or PCR) from within the past 72 hours. Results from at-home tests are not accepted.
If you'd like to use an on-site rapid testing service, COVID-19 Antigen Rapid Self-Test at Home Kit available for purchase at $75/kit.
Alternatively, you can go to local covid-19 testing facilities in Morgan Hill to perform the test or purchase testing kit.
- We will be requiring proof of negative covid-19 test result to enter OT Park for each individual above 12 years of age OR proof of vaccination.
Do visitors ages 5 to 11 need to show proof of vaccine or a negative COVID-19 test?
No. At this time, we are requiring proof of vaccination or a negative COVID-19 test only for visitors ages 12 and up.
What is accepted as proof of vaccination?
Valid forms of vaccination records include:
- Original CDC vaccination record (card or similar document from another state, local, or foreign government). We accept any vaccine approved by the World Health Organization.
- Photocopy of original vaccination record
- Electronic or digital photo of original vaccination record
- the CLEAR digital vaccine card. Please see FAQ below for instructions
What kinds of COVID-19 tests are accepted?
We will accept negative COVID-19 test results via lab-run polymerase chain reaction (PCR) or rapid antigen test. We will not accept results from at-home tests. Proof of a negative COVID-19 test must include name, date and type of test, and indication that the result was negative for COVID-19.
The test must be taken within 72 hours of your visit to OT Park. Visitors ages 18 years and over will also be required to provide a matching photo ID.
Where can I get a COVID-19 test before I visit?
You can visit local covid-19 testing centers.
Using the CLEAR digital vaccine card for express entry
What is the CLEAR digital vaccine card?
You can enjoy faster entry to OT Park with a digital vaccine card from CLEAR. This serves as both identification and proof of vaccination and gives you access to our express entry line.
How can I get the CLEAR digital vaccine card?

Can kids use the CLEAR digital vaccine card?
Yes! Youth ages 14 to 17 with their own smartphone and email address can use CLEAR’s digital vaccine card with the help of a parent or guardian. Each individual must have their own smartphone in order to use the CLEAR digital vaccine card.
Youth ages 12 to 17 without a smartphone may accompany participating adults as long as they have their proof of vaccination or negative test results.
How to set up a CLEAR account for a youth:
- Downloaded the CLEAR app
- Click the account icon on the bottom right hand corner
- Select “Minor Accounts”
- Go through the enrollment process and add in the youth’s email address, set up their account, and have them snap a selfie!
Once you’ve set up their account, the youth will then:
- Download the CLEAR on their smartphone
- Log in with the email and password set up by their parent or guardian
- Click on the blue digital vaccine card tile
- Add their proof of vaccine through the CDC card upload option
Does it cost me anything to use the CLEAR digital vaccine card?
CLEAR’s digital vaccine card is free for you to use on the CLEAR app.
How do I find my digital vaccine card with CLEAR?
The digital vaccine card will be available to you after you’ve downloaded the CLEAR application and added your vaccine information. To access your digital vaccine card on your smartphone, open your CLEAR app and scroll down to a blue card called “Digital vaccine card.”
I will be visiting with kids ages 12 to 17 who are vaccinated but don’t have a smartphone. Can we use the express entry line?
Yes. In order for a group to use the express entry line, all visitors ages 18 and over must have registered for their CLEAR digital vaccine card and be able to display it on a smartphone. Visitors ages 12 to 17 are not required to show an ID and may accompany participating adults as long as they have their proof of vaccination or negative test results.

What is Occupational Therapy and how it can help you?
Occupational therapy is the only profession that helps people across the lifespan to do the things they want and need to do through the therapeutic use of daily activities (occupations). Occupational therapy practitioners enable people of all ages to live life to its fullest by helping them promote health, and prevent—or live better with—injury, illness, or disability.
Common occupational therapy interventions include helping children with disabilities to participate fully in school and social situations, helping people recovering from injury to regain skills, and providing supports for older adults experiencing physical and cognitive changes.
Occupational therapy services typically include:
- an individualized evaluation, during which the client/family and occupational therapist determine the person’s goals,
- customized intervention to improve the person’s ability to perform daily activities and reach the goals, and
- an outcomes evaluation to ensure that the goals are being met and/or make changes to the intervention plan.
Occupational therapy practitioners have a holistic perspective, in which the focus is on adapting the environment and/or task to fit the person, and the person is an integral part of the therapy team. It is an evidence-based practice deeply rooted in science.
Do I need a prescription from a doctor before starting occupational therapy?
No. You don't need your child's physician's referral for our private pay services.

What is OT Park?
OT Park Box is a monthly kids’ skilled activity subscription box that sends activity ideas and materials to your doorstep! Each month, tiny fingers will discover carefully curated activities with its materials needed, an informative activity guide for parents written by an experienced pediatric occupational therapist, and stickers for fun! Every box includes a book for you and your child to read because we at OT Park, firmly believe in the magic of reading!

What Ages do you Treat?
We specialize in treating children from birth to 6 years. However we do treat some older children with specialized challenges. Please contact us directly if you have an older child.
What can I expect from an Evaluation?
In the initial evaluation, the therapist will perform an assessment to identify how we can best meet your and your physician’s concerns. The therapist will then discuss the results with you and review the recommended treatment options. If the therapist recommends physical or occupational therapy, you will be given instructions about how to schedule appointments. We will also communicate our findings with your child’s referring physician or certified nurse practitioner.
How long is each session?
Recommendations for the appropriate frequency of treatment sessions will be given by the treating therapist and may change over time, depending on the needs of the child and family. Although we will share our best clinical opinions, we respect and value your perspective as a parent.
Do you accept health insurance?
Although we are not providers for any insurance companies at this moment and do not accept payment from companies for services, we have had many families over the years that have been able to get some level of reimbursement for therapy. We highly recommend that all families who will be submitting claims for insurance coverage contact their provider to find out what their specific plan covers and the process required to get coverage.
Your monthly invoice will include dates of service, CPT and diagnostic codes. When needed, we will gladly share pertinent information about our therapy services (plan of care and/or completion of insurance forms) at no charge. However, for requests that require more than 15 minutes of time, an additional charge will be incurred.
Feedback from families over the years has indicated that the keys to insurance reimbursement are:
- Understanding your policy
- Keeping notes on dates/times/contact person/information shared for any communication with your company
- Following up and not giving up
- The squeaky door gets fixed!
How does payment work?
- We bill at the time of the beginning of the session. You can also opt in for our payment package options to pay in advance and get great discounts.

Can I photograph or video-graph sessions?
Please ask before taking a photo or recording. You may be asked to wait until a task is finished, or to assure that no other patients are in the area, or to allow the therapist to step out of the picture. Photos and videos of therapy should not be posted on social media sites such as Facebook.
Do I need to be present for my child’s therapy session?
A parent or legal guardian must be present at your child’s initial therapy evaluation; this is important so that you can give consent for your child to be treated. Being present at the evaluation will give the therapist a chance to speak with you about your concerns and goals for your child.
The most important thing you can do for your child is to make sure a parent or caregiver attends every therapy session, even if your child is a teenager. One of our primary goals is to give your entire family skills and knowledge to help your child gain the greatest benefit from therapy.
If absolutely necessary, teens may attend their therapy sessions without their legal guardian present, provided they are responsible young adults and the therapist is in agreement.

What's in the box?
Every month you will receive a surprise assortment of many different age-appropriate activities with a detailed instruction booklet - that is our own activity guide designed for that box. Examples include activity worksheets, pencils, sensory toys, activity cards, play-doh, colors, putty, a kid’s book, infant’s mobile, wooden toys, textured tactile toys/ fidgets, craft activities that promote gross and fine motor skills, sensory-motor, handwriting, visual motor and activities of daily living skills, and much more!

What if my child is unable to do the activities?
This is not unusual - and the OT Park Box is designed to account for it. If your child is unable to perform any of the activities mentioned in the box, then take a break, and try repeating it in a day or two. Children at this age normally learn through repetition, and it is when your child repeatedly engages in the activity and the skills it uses, that your child is actually developing!
Who can benefit from The OT Park Box?
We offer boxes and subscriptions for a wide range of ages and interests, from infants (0 months) through toddlers of up to 3 years.
What age group are these boxes for?
0-3 years old. However, this is just a general guideline as our children with special needs have different levels of independence and various learning levels and developmental abilities. You may need to provide more guidance & assistance to your child for certain contents in the box (e.g., a craft/ fine motor based activity.) However, many of the items in OT Park Box are appropriate for children of the ages that the box suggests.
Can my child use the ot park box independently?
No. We strictly recommend OT Park Box activities must be performed under parental supervision.
Is this a therapy box for kids?
OT Park is NOT a therapeutic box. We do not provide therapy in the box. These boxes are curated solely for the purpose of fun, enjoyment, and practice of various skills that are appropriate for specific age group depending on your subscription.

An item in my box broke. What Can I do?
We always check the items to make sure they are intact before placing them inside your OT Park Box. However, if you received a broken item, please email us at firstname.lastname@example.org within three (3) days upon receiving your OT Park Box, attached with a photo of the broken item, and we will gladly assist you! Unfortunately, we cannot be responsible for items that are broken during and after use.
How much does an OT Park box Subscription cost?
An OT Park Box starts at $74.99/month + Free Shipping within USA + Tax.
If I sign up for a subscription today, when will I receive my first OT Park Box?
Your first box will begin processing immediately after your order is placed. Though here is some information a customer may note:
If you have signed up before 25th of any month, you will receive the current month’s box. Signing up after 25 to end of the month, will be eligible for the next month’s box.
Example: You signed up on 20th of september. You will receive a September box.
But, if you signed up on 28th of the month, you will receive an October box when October boxes ship from the warehouse. (Boxes are shipped usually between 15-25 of each month on three different groups). You will receive an email notification on when your box is shipped.
Once shipped, your box will take approximately 3-10 business days to arrive at your door depending on the location.
When will I get my first OT Park Box If I sign up for pre-order?
We are closed for Pre-orders now.
Can I cancel my subscription and receive a refund?
You can cancel anytime by logging into your account and click ‘Cancel my subscription’. In order to receive a refund, you must request one before the 25th of the month. Unfortunately, we are unable to provide refunds for boxes that have already shipped or if it is past the 25th of the month. To request a refund, please email us at email@example.com
How do I sign up again after having previously canceled?
If you would like to reactivate a previously canceled or expired subscription, you can go back into your Subscription Details page and click “Reactivate”. If you’d like to start a new subscription, simply add the new subscription to your cart and proceed through the checkout.

Is Shipping Free?

Can I order just one time box?
Yes you can. Please select “gift a box” or “One time purchase” in the drop down menu of your box category page.
What payment options are available?
We accept all online modes of payment including Debit/Credit Cards, PayPal, Apple Pay or Amazon Pay.
Do you offer Gift cards?
Yes we do. Please visit our shop. If you do not find that option, Please reach out to email@example.com to receive/send a gift card - we are happy to help.
What do I do if I accidentally signed up for more than one subscription?
Contact our customer service team who would be happy to look into your account for you. A refund will be given when needed. Please contact us. You will receive a reply within 2-3 business days.
How can I change my subscription type from one age group to another?
We offer a six-month subscription for each age
If you are month to month We offer a 6 months subscription for each age group. When you are receiving your subscription for a specific age-group for example 6-12 months box group, and you are in the 12th month, you can email us as firstname.lastname@example.org and request change or sign up directly on the website for the next 6 months by selecting the ‘get started’ button to continue services for the next 6 months.
I didnt receive an order confirmation for my subscription. What Do I Do?
No worries! Log in to your account and click the Order Number to get the breakdown of your order as well as its status.
Why is my credit card being declined?
Your credit card may be declined because we do not accept the form of payment you are trying to enter. Also, double check that the billing information you are entering matches with the billing address connected to your credit card. If you are still having trouble, contact us so we can help you out.
Account and Billing
When will I be billed for my OT Park Box Subscription?
If this is your first time signing up for OT Park Box, you will be billed immediately except for the launch offer (Launch offer- you will be billed on Aug 31st to get the first box in September). For all subsequent renewals, you will be billed on the same date of the next month. For example, if you placed your first order on April 2ndth your next billing would be on May 2nd, then June 2nd,etc.
Will I receive the current month’s OT Park Box when I sign-up? How does the renewal system work overall at OT Park?
Yes, You will get a box for the current month if you sign up between 1-25th of any month. You account renews at the same date of your sign-up. For example, if you signed up on 16th of October, your account will be renewed on 16th of November and so on. You can cancel anytime. We do not provide refunds for the box that is already shipped.
Do you offer payment plans?
Sorry at this time, we do not offer payment plans.
Am I able to skip a month without losing my subscription?
Of course you can. Just contact our customer service or email at email@example.com. We will take care of it.
Can I change frequency of my box delivery?
Sorry at this time, we do not provide such option.
How Can I change my credit card number?
Simply login to your account. Go to my profile and update payment information.
Shipping & Tracking
How do billing and shipping work?
We are a monthly subscription. That means you are re-billed every month. When you purchase your first box, your card is charged immediately (that same day). You will be re-billed for your next box on the same date of the next month.
Boxes are shipped out on 15-25th of each month. You will receive the current month’s box whenever you sign up. For example, in order to receive your “September box”, you must sign up before the 25th of September.
To receive the "current" box, you must sign up on or before the25th of the previous month. For example, in order to get the "October's box", you must sign up before the 25th of October.If you still want previous box, you can email us at firstname.lastname@example.org or visit our shop on our website.
What countries do you send OT Park box?
The OT Park Box currently supports free shipping within The United States of America till Dec.2020. We currently ship every other country. Please contact email@example.com and we will provide you with information.
Do you offer expedited shipping?
Sorry at this time, we do not offer expedite shipping.
How long will it take for my OT Park Box order to arrive?
Once your box has shipped, expected delivery times in the United States are on average 3-10 business days, depending on the location.
How Do I track my order?
Once your order has been shipped, you will receive a shipping confirmation email containing the tracking information you need. This number can also be found in your profile information on our website.
My tracking number isn't updating. what does this mean?
The tracking information will be updated 24-48 hours after receiving a notification that your order has been shipped. If the time period passes and an update is still not available, please contact customer service at firstname.lastname@example.org with your order ID so we can assist you.
Its been 10 days since my box shipped and I still haven't received it. What should I do?
We are so sorry to hear your box has not arrived yet! Please contact customer service so we can verify that your address is correct and take any action needed to get you your box!
How do I update my shipping address?
Log in to your OT Park account and edit your information for each subscription under “My Subscriptions” in your profile. Please note that your shipping address must be updated before your renewal date in order for the package to go to that new location. If you require assistance updating your shipping address, please contact us!
